Background: Garbhasrava (early pregnancy loss) is a distressing reproductive health issue affecting approximately 10-15% of clinically recognized pregnancies. In Ayurveda, it is attributed to Garbhashaya Dosha (uterine disorders), Beeja Dushti (genetic abnormalities), and Vata-Pitta imbalance leading to improper nourishment of the developing embryo. The Prajasthapana Mahakashaya group of herbs, described by Acharya Charaka, is specifically indicated for reproductive health and pregnancy maintenance. Aim: To evaluate the pre-clinical safety and clinical efficacy of Prajasthapana Mahakashaya Granules along with Vatashunga Nasya in the prevention of Garbhasrava in high-risk pregnancies. Method: An open-labeled, randomized, prospective controlled clinical trial will be conducted on 80 pregnant women with a history of recurrent pregnancy loss or threatened abortion. Participants will be randomized into two groups: Group A will receive Prajasthapana Mahakashaya Granules (5g twice daily) with Vatashunga Nasya (2 drops each nostril daily), while Group B will receive standard supportive care for 12 weeks or until the completion of the first trimester. Assessment will be conducted using USG parameters, serum β-hCG levels, progesterone levels, and clinical symptom scoring. Result: Based on previous research, Prajasthapana Mahakashaya herbs possess Garbhasthapaka (embryo-supporting), Garbhashaya Shodhana (uterine cleansing), and Vata-Pitta balancing properties. Vatashunga Nasya is expected to provide Vatanulomana (downward movement of Vata) and enhance uterine perfusion. Conclusion: Prajasthapana Mahakashaya Granules with Vatashunga Nasya offer a safe, comprehensive, and holistic approach for the prevention of Garbhasrava, potentially improving pregnancy outcomes in high-risk women.
