OpenFFR v0 — Founding documentation package for the Open Fragility Fracture Registry, an open international, federated, non-profit clinical registry of fragility fractures, an initiative of FFN — Fragility Fracture Network Portugal, Associação. This package contains nine documents constituting the initial design of the registry: institutional one-pagers (PT/EN), the protocol for institutional adhesion, the internal regulation, the data dictionary, the founding minutes template, the indicator catalogue and its companion clinical manual, and the Cognitive & Functional Baseline Assessment Form (CFBAF). The CFBAF is the distinguishing element of OpenFFR: a structured instrument for harmonised capture of pre-fracture cognitive, functional and frailty status, integrating five validated scales (categorical cognitive status, IQCODE-SF, 4AT, NMS, CFS Rockwood) with explicit application and scoring rules.
