2829-LB: STEP UP T2D: Participants Achieving BMI, WHtR, and Cardiometabolic Treatment Targets

Introduction and Objective: A BMI 27 kg/m2 and waist-to-height ratio (WHtR) 0.53 have been proposed as potential treatment targets for obesity management. In this analysis of the phase 3 STEP UP T2D trial (NCT05649137), we examined the proportions of participants (pts) with obesity and T2D who achieved BMI and WHtR targets and cardiometabolic outcomes while on semaglutide (sema) 7.2 mg, sema 2.4 mg, or placebo (pbo). Methods: The proportions of pts achieving BMI 27 kg/m2 and WHtR 0.53 targets, and low-risk thresholds for cardiometabolic outcomes (normoglycemia, HDL-cholesterol, blood pressure, and triglycerides) at week 72 were assessed. Data are descriptive from the on-treatment period. Results: A total of 512 pts (sema 7.2 mg n=307, sema 2.4 mg n=103, and pbo n=102) with mean (SD) T2D duration of 8.4 (6.4) years and HbA1c of 8.1% (0.9) were included. At baseline, no pts met BMI and/or WHtR targets. At week 72, greater numerical proportions of pts receiving sema 7.2 mg achieved BMI and/or WHtR targets, and low-risk cardiometabolic outcomes compared with those receiving sema 2.4 mg or placebo (Figure). Conclusion: Consistent with STEP UP (NCT05646706), numerically greater proportions of pts achieved BMI and/or WHtR targets, and cardiometabolic threshold outcomes with sema 7.2 mg versus sema 2.4 mg and pbo in STEP UP T2D. However, overall target achievement was lower than in STEP UP. Therefore, an unmet need remains in this population with increased CVD risk.
