Introduction Hysterectomy remains one of the most frequently performed surgical procedures in women’s health. The choice of surgical approach has evolved considerably with technological and clinical developments. Understanding the comparative advances, indications, and outcomes is essential for optimizing patient care. Objective: The aim of this narrative review is to examine and compare the advances, clinical indications, and outcomes associated with abdominal, vaginal, and laparoscopic hysterectomy techniques. Methods: A comprehensive narrative review was conducted by surveying recent literature evaluating surgical approaches to hysterectomy, focusing on clinical trials, cohort studies, and expert consensus.Results and Discussion: We present a synthesis of current evidence highlighting the evolution of each approach, clinical decision pathways, perioperative outcomes, and patient-centered results. Conclusion: The review underscores that minimally invasive techniques, when feasible, tend to offer improved recovery profiles, though approach selection remains highly dependent on patient factors and surgeon expertise.
