Abstract Case summary: We present the case of a 34-year-old woman who experienced a cataclysmic spontaneous coronary artery dissection (SCAD) during the postpartum period. A multidisciplinary strategy was employed, integrating intracoronary imaging and computed tomography angiography to guide clinical decision-making. These modalities enabled a carefully timed, complex percutaneous coronary intervention (PCI) performed six months after initial presentation, ultimately leading to a favourable clinical outcome
