Background: Myocardial fibrosis (MF), a critical pathological process in various cardiovascular diseases, has gained increasing attention for traditional Chinese medicine (TCM) interventions. This study analyzed research landscape, developmental characteristics, and frontiers in TCM-based MF management from 2008 to 2025 through bibliometric analysis. Methods: Publications were retrieved from the Web of Science Core Collection (WOSCC) using the search formula: “(TS OR TI OR AB OR AK = MF) AND (TS OR TI OR AB OR AK = TCM)” or the period January 1, 2008, to April 9, 2025. CiteSpace, VOSviewer, SCImago Graphica, Bibliometrix R Package, and Excel were utilized for analysis. Results: Annual publication output demonstrated phased growth patterns. China was identified as the most productive country in terms of publication volume and citation frequency. Dynamic shifts in research teams were observed, with Zhou Hua from Guangzhou University of Chinese Medicine emerging as a prominent contributor. Journal of Ethnopharmacology was recognized as the leading journal based on publication quantity and academic influence. Keyword analysis revealed heart failure, TCM, and MF as the top 3 research foci. Conclusions: Current research hotspots concentrate on 3 domains: pharmacological actions of herbal bioactive components, network pharmacology-based mechanistic exploration, and clinical translation of integrative medicine approaches. These advancements not only provide novel research perspectives for TCM-based MF interventions but also facilitate the integration of complementary and alternative medicine with modern medical paradigms. The findings highlight TCM’s expanding clinical applications in cardiovascular therapeutics and its potential to bridge traditional knowledge with contemporary biomedical innovations.
