Abstract Intradural extramedullary capillary hemangiomas of the cauda equina are very rare. Radiographically, they may mimic other intradural tumors like meningioma or nerve sheath tumors. Here, we present a case of a young woman who presented with low back and left leg radicular pain. Magnetic resonance imaging showed an enhancing lesion in the intradural space at the L4/L5 level. Initially, a diagnosis of meningioma with differential diagnosis of schwannoma was suspected; however, histopathology revealed it to be a capillary hemangioma. Although rare, because of its similar clinico-radiographical features to other common intradural tumors and high risk of intraoperative bleeding, capillary hemangioma should be included in the differential diagnosis when planning surgical excision.
