Abstract This case report describes the successful surgical management of external inflammatory root resorption (EIRR) on a maxillary right central incisor in a 38-year-old male. The patient presented with discolouration, pain and mobility of the affected tooth, with a history of trauma and previous root canal treatment. Cone-beam computed tomography confirmed the resorptive defect. Treatment involved non-surgical endodontic therapy, followed by surgical repair using Bio-C root repair material and platelet-rich fibrin scaffold. Postoperatively, the patient presented reduced symptoms and improved periodontal health and stability, highlighting the efficacy of combining biomaterials and regenerative techniques in EIRR management.
