A 67-year-old woman had severe refractory pincer nail deformity affecting all fingernails, which failed multiple therapies, including repeated surgical avulsions, antifungals and topical steroids. She subsequently underwent electron beam radiotherapy, resulting in significant pain relief and functional improvement. This case highlights radiotherapy as a potential novel intervention for treatment-resistant pincer nail deformity.
