Case summary A 27-year-old professional cyclist presented with exercise intolerance following a viral illness. High-sensitivity troponin I was markedly elevated, while high-sensitivity troponin T remained normal. Major cardiac causes were excluded, raising suspicion for macrotroponin complexes.
