The novel AccuSafe guidewire facilitated successful transseptal puncture for AF ablation in a patient with complex post-operative atrial septal anatomy.
Post-repair atrial septal anatomical variations may increase transseptal puncture difficulty. While the novel AccuSafe guidewire shows promise as a procedural tool, its utility in such complex cases requires further evidence-based validation. We report a case of a 73-year-old female who developed atrial fibrillation (AF) following surgical repair of an atrial septal defect (ASD). An initial ablation attempt at another institution had failed. The patient presented with recurrent palpitations, concurrent heart failure, and anxiety. At our center, the AccuSafe transseptal puncture guidewire was successfully used to traverse the interatrial septum, enabling the completion of radiofrequency ablation for AF. The AccuSafe puncture guidewire was successfully applied and showed potential value in this individual case of post-operative ASD repair.
