Abstract Dhat syndrome, a culture-bound disorder prevalent among South Asian populations, is characterized by excessive concern over semen loss, which leads to distress and somatic symptoms. While typical presentations include perceived semen loss through urine, nocturnal emissions, or masturbation, atypical cases may involve more unconventional routes. We present the rare case of a 42-year-old man who misattributed his sputum to semen loss, resulting in psychosomatic distress. Management included psychoeducation, cognitive restructuring, and relaxation techniques.
