Background: Retained intradural spinal catheter fragments are rare but potentially serious complications that may lead to neurological injury, infection, and catheter migration. Case Description: Herein, we describe a fluoroscopy-guided surgical technique to retrieve a retained intradural spinal catheter in a patient with Marfan syndrome and dural ectasia following major aortic surgery. Intraoperative fluoroscopy enabled precise localization and targeted exposure and safe catheter removal while minimizing neural manipulation. Complete retrieval was confirmed, and the patient experienced no postoperative neurological deficits or complications. Conclusion: Early image-guided intervention may help prevent catheter migration, infection, and delayed neurological complications in selected patients.
