Key Points

  • This research aims to explore the reliability of elevated thyroglobulin washout in diagnosing thyroid cancer.
  • Presented two case reports of patients with elevated thyroglobulin washout
  • Documented patient history including iron-handling disorders
  • Analyzed histopathological findings of cervical lymph nodes
  • Both cases had benign lymphadenopathy despite high thyroglobulin levels
  • Iron-handling disorders may complicate the interpretation of thyroglobulin levels
  • Findings indicate the need for multimodal interpretation in clinical practice

Abstract

Markedly elevated thyroglobulin (Tg) washout is often considered highly suggestive of metastatic differentiated thyroid cancer. We present two cases of false-positive Tg washout in cervical lymph nodes with benign pathology, notably in patients who were carriers of iron-handling disorders. These findings highlight the limitations of Tg washout as a standalone marker and the need for multimodal interpretation.
