Abstract The increasing prevalence of chronic diseases worldwide has necessitated innovative approaches to healthcare delivery, particularly in primary care settings. Digital health interventions have emerged as promising solutions to address the growing burden of chronic disease management while improving patient outcomes and reducing healthcare costs. This review examines the current landscape of digital health technologies in primary care chronic disease management, including mobile health applications, telemedicine platforms, remote monitoring devices, and artificial intelligence-driven tools. Evidence suggests that digital health interventions can significantly improve patient engagement, medication adherence, clinical outcomes, and quality of life for patients with diabetes, hypertension, cardiovascular disease, and other chronic conditions. However, implementation challenges persist, including digital literacy barriers, data privacy concerns, healthcare provider resistance, and integration issues with existing healthcare systems. The coronavirus disease 2019 pandemic has accelerated the adoption of digital health technologies, creating opportunities for sustained integration into routine primary care practice. Future directions should focus on addressing implementation barriers, ensuring equitable access across diverse population, developing robust evidence bases through rigorous clinical trials, and establishing standardized frameworks for digital health intervention evaluation. Primary care providers and policymakers must collaborate to harness the potential of digital health technologies while addressing associated challenges to optimize chronic disease management and improve population health outcomes.
