Testicular adrenal residual tumors (TARTs) are a common complication in males with congenital adrenal hyperplasia (CAH) and have been well described in patients with CAH caused by 21-hydroxylase deficiency (21-OHD), but TARTs with 11β-hydroxylase deficiency (11β-OHD) are extremely rare with few reports. We report a 9-year-old male with TARTs and his clinical course. For pediatric giant 11β-OHD-related testicular adrenocortical tumors, early diagnosis and standardized treatment can effectively reduce tumor size, indicating that early damage to testicular function is reversible. Ultrasound plays a significant role in the diagnosis and follow-up of TARTs.
