Abstract Endometriomas, also referred to as ovarian chocolate cysts, are a serious health risk for a large number of women globally. An innovative and all-encompassing method of treating these cysts is provided by unani medicine in conjunction with lifestyle adjustments. This case study describes the effective treatment of an ovarian chocolate cyst using an integrative method that combines lifestyle changes with Unani therapy. A 26-year-old woman arrived with a history of irregular menstrual cycles, chronic pelvic pain, and primary infertility. An extensive chocolate cyst on the ovary was discovered via ultrasound. After taking consent from the patient was treated with Unani medicine, which includes Ma‘jūn Dabīd-ul-ward, Arq-i-Mako, Arq-i-kāsnī , and Niswānī , which are already well-known for their ability to regulate hormones and reduce inflammation of the visceras. The patient received instructions on adhering to the management and correct administration. Certain food limitations were also put in place to prevent aggravating hormonal imbalances that are linked to the development of cysts. In an effort to enhance general well-being and facilitate the clearance of the ovarian chocolate cyst, brisk walking was stressed. Following a committed three months of following the Unani management and lifestyle change plan, the patient reported a notable decrease in irregular menstrual periods and pelvic pain. A follow-up ultrasound showed a lesion-free, normal ovary, indicating a successful remission without the need for contemporary medication or surgery. This case study demonstrates the possible efficacy of an integrative strategy that includes lifestyle change and Unani medicinal therapy in the treatment of ovarian chocolate cysts. Although more specific investigation and larger studies are necessary to confirm these results, this example highlights the importance of focusing on patient-centered care and exploring alternative non-invasive treatment management for ovarian cysts.
