Abstract Oseltamivir is a viral neuraminidase enzyme inhibitor commonly prescribed for the prevention and management of influenza. Common adverse effects of oseltamivir include gastrointestinal side effects such as nausea, vomiting, diarrhea, headache, insomnia, and skin rashes. It has been associated with various neuropsychiatric side effects such as delirium, delusion, and aberrant behavior in individual case reports. The overall incidence and clinical patterns of these adverse effects remain poorly characterized and underreported in the literature. In this report, we present the occurrence of neuropsychiatric symptoms, such as aggressive behavior, restlessness, paranoid ideas, and insomnia, following the administration of oseltamivir in a middle-aged male patient. These symptoms were later resolved after discontinuing the drug.
