Abstract The World Health Organization Mental Health (MH) Atlas 2024 provides a global snapshot of MH systems across 144 countries, highlighting persistent gaps in financing, workforce, and community-based care between geographical regions and the World Bank income groups, i.e. Low-Income Countries, lower-middle-income countries (LMICs), upper-middle-income countries, and high-income countries. The National Mental Health Survey of India 2015–16 offered an in-depth, state-level systems assessment, revealing analogous deficits within the Indian context. This short communication juxtaposes these two landmark studies to delineate converging patterns, contextual differences, and pathways for systems reform. The comparison reveals India’s alignment with LMIC trends in underfunding, limited workforce density, and inadequate data systems, while also identifying areas of progress in legislative reform and digital integration. We recommend a Comparative Mental Health System Index as a 5-point index to compare these two landmark reports. As India awaits the next NMHP, a synthesis of both reports underscores the urgent need for strategic investment, human resource scaling, and evidence-based community integration.
