Abstract Introduction Since HNS became covered by insurance in June 2021on Japan, over four years have passed. As of July 2025, the total number of HNS implant patients in Japan has reached 104 cases. Among these, the total number of patients treated at Meitetsu Hospital has reached 31 cases, and the number of completed Fine Tune PSG procedures has risen to 18 cases. Methods In the retrospective study, we examined the distribution of age, sex, and BMI among these 18 patients and also investigated changes in AHI and L-SPO2 before and after surgery. Results Fine Tune PSG Completion Cases: 18 Cases Age Distribution: 70s: 13%, 60s: 19%, 50s: 32%, with those aged 50 and above accounting for 64%. The male-to-female ratio was 94% male and 6% female. The mean BMI was 25.5 ± 4.8. AHI decreased significantly from 43.7 ± 14.1 to 14.4 ± 17.4. The Green rate (AHI 15) reached 89%. (P 0.01) L-SPO2 significantly improved from 74.8 ±6.0% to 85.7±8.8%. (P 0.001) Conclusion This time, the rate of improvement to an AHI below 15 at Meitetsu Hospital reached 89%. This suggests that even for Japanese patients in Asia with CPAP intolerance or non-compliance, if they are convinced of and desire the surgery despite its risks and disadvantages, it can be considered a treatment worth actively pursuing. Furthermore, given that 64% of patients undergoing surgery were aged 50 or older, this treatment may be particularly suitable for the elderly. Support (if any)
