2 children (a 14-year-old male and a 9-year-old female) with structurally normal hearts presenting with ventricular fibrillation cardiac arrest

Intervention

Sprint/burst exercise stress testing (Sydney ASAP EST protocol) and invasive electrophysiology study with radiofrequency ablation

Comparator

Standard Bruce protocol exercise stress testing

Outcome

Provocation of ventricular ectopy/tachycardia and successful identification of arrhythmia substrate

Sprint/burst exercise stress testing can unmask adrenergically-mediated ventricular arrhythmias originating from the left posterior fascicle in children with unexplained VF arrest and structurally normal hearts.
