Hemodynamic Effects of Remimazolam Versus Propofol in Catheter Ablation Under General Anesthesia

Does remimazolam reduce the duration of intraoperative hypotension compared to propofol in adults undergoing catheter ablation under general anesthesia?

Population

Adults aged 18 years or older undergoing catheter ablation under total intravenous anesthesia, n=59 (propofol n=32, remimazolam n=27). Mean age 63.1 years, 78.0% male. Key comorbidities: hypertension (69.5%), heart failure (30.5%), atrial fibrillation (74.6%).

Intervention

Remimazolam (bolus 1-2 mg/kg; continuous infusion 1-2 mg/kg/h) titrated to maintain bispectral index between 40 and 60, coadministered with remifentanil and rocuronium.

Comparator

Propofol (target-controlled infusion, effect-site concentration 2-3 mg/mL) titrated to maintain bispectral index between 40 and 60, coadministered with remifentanil and rocuronium.

Outcome

Cumulative duration of mean arterial pressure (MAP) < 65 mm Hg during anesthesia.safety

Remimazolam does not appear to offer a significant hemodynamic advantage over propofol in reducing the duration of hypotension during catheter ablation under general anesthesia.
