Endoscopic proximal hamstring repair represents an evolution in the surgical management of posterior hip pathology. Mastery of this technique requires progression beyond intra-articular and peritrochanteric hip arthroscopy into the technically demanding subgluteal space. In experienced hands, the endoscopic approach offers improved visualization, enhanced control, and more reliable identification and protection of the sciatic nerve. Compared with traditional open techniques, it allows for greater precision and procedural efficiency while reducing dependence on exposure and assistance. As surgical experience and outcome data continue to mature, endoscopic repair is increasingly recognized as a reproducible and safe option in appropriately selected patients. The transition from open to endoscopic management reflects the natural evolution of modern hip preservation surgery.
