A system that is continuously available, longitudinal, guideline-aware, and patient-facing could, in principle, help close gaps in diagnosis, treatment initiation, medication titration, monitoring, and follow-up. Whether such systems can do so safely, equitably, and in ways that strengthen rather than erode the clinician-patient relationship remains an open question.

I'm not sure if turning an interventional cardiologist into a 'human QCA machine' is necessarily the answer. I think the real opportunity is to understand why and when discrepancies may occur that would change clinical decision making and outcomes.

The AHA guidelines say we should be giving heparin to patients undergoing revascularization or receiving fibrinolytics, however all their recommendations are evidence level C, which translates to 'only consensus opinion of experts, case studies, or standard-of-care'. In other words, we really don't know.
