In this study prospective RCT (NCT03434132), we assessed early HRQoL modifications between patients receiving Ileal Conduit (IC) or orthotopic Padua Ileal Bladder (PIB). METHODS: Patients were eligible if they had a diagnostic TURBt with cT2-4cN0cM0, or recurrent HG NMIBC and no anesthesiologic contraindications to robotic surgery. Between Jan 2018 and Sept 2020, patients were enrolled with a covariate adaptive randomization process based on: BMI, ASA, baseline Hgb, UD, neoadjuvant chemo and cTstage. Data from EORTC QLQ-C30 and QLQ-BLM30 questionnaires were collected. Normally distributed HRQoL continuous variables were compared with paired t test. A generalized linear mixed effect regression (GLMER) model including a random effect at patients level was implemented to test variation of HRQoL items along time and between groups. RESULTS: Out of 116 patients enrolled, 106 reached at least 6 mo of fup. Baseline outcomes were reported Table 1. Patients who underwent IC were older and with a higher ASA score (p<0.034). Peri- and post-operative outcomes were comparable between groups, except for operative time (p=0.032). At 6 mo HRQoL evaluation, both cohorts reported significant worsening in terms of physical, and sexual functioning, with a significant impairment of fatigue, dyspnea, constipation and body image (all p<0.025). Patients receiving PIB were also more likely to report impairment of role, social functioning, appetite loss, diarrhea and financial difficulties (all p<0.02). At GLMER analysis PIB cohort displayed a significant impairment in terms of social functioning and financial difficulties, while IC cohort experienced a significant impact of sexual functioning (p<0.041, Table 3). CONCLUSIONS: Despite comparable global health status, patients receiving PIB are more likely to experience favourable sexual function recovery but significant social function impairment and financial difficulties. Preoperative counselling for patients selected for PIB should discuss longer postoperative recovery and social functioning.
