Glioblastoma is the most common and aggressive primary brain tumor in adults. However, despite its aggressive nature, dural-based progression remains exceedingly rare. This rarity is partially due to the protective effects of the blood-brain barrier and the poor survival of affected patients. As a result, the pathophysiology and imaging characteristics of dural-based glioblastoma progression remain poorly understood and underreported. In this case series, we present 4 cases of glioblastoma with dural-based progression and review the relevant literature. We highlight the MRI imaging features of dural progression and explore potential pathophysiology, diagnosis, and management in this rare manifestation.
