The PRO-TAVI trial randomized 466 older patients (median age 81) with severe aortic stenosis and coronary artery disease to PCI before TAVI or deferral of PCI, finding that deferral was noninferior for the composite of death, MI, stroke, or major bleeding at 1 year (24.1% vs 25.8%; P<0.001 for noninferiority). Deferral was associated with significantly less major bleeding (6.2% vs 14.8%), and only 11% of deferred patients ultimately required PCI. Reaction has been generally supportive of the findings but with important caveats about applicability to younger, lower-risk populations and the contrast with the NOTION-3 trial which favored PCI.
