Cross-sectional study compares sensitivity of ultrasonography and MRCP to ERCP in patients with cholestasis, indicating MRCP's superior reliability.
Abstract Background This study compared USG and MRCP findings with ERCP results in patients with biliary tract disease and investigated their diagnostic reliability. Methods This cross-sectional study included 158 patients who presented with abdominal complaints. Those evaluated by USG and MRCP who then underwent therapeutic ERCP were included. Results The patients' mean age was 60.3 ± 18.4 years (range: 18–93); 66 (41.8%) were male. The most common diagnosis of ERCP was bile duct stones (57.9%). USG sensitivity/specificity vs ERCP: 50%/69.1%. MRCP sensitivity/specificity vs ERCP for CBD stones: 78.9%/44.1%. Conclusions Our findings indicate that the diagnostic sensitivity of USG is not high, whereas MRCP shows high sensitivity, especially for common bile duct stones. USG is a primary imaging modality; MRCP serves as crucial follow-up when the USG is insufficient. ERCP remains the predominant therapeutic intervention.
