Supply chains across industries are undergoing a significant digital transformation, with advanced technologies such as the Internet of Things (IoT) and blockchain playing a pivotal role in driving innovation. This research presents a comprehensive framework that integrates IoT-enabled sensing and monitoring with blockchain-based secure data management to achieve end-to-end supply chain automation. In the proposed model, IoT devices deployed at various supply chain checkpoints collect real-time operational data, enabling continuous visibility into inventory, transportation, and environmental conditions. The blockchain layer ensures transparent, tamper-proof data recording and facilitates automated settlements through smart contracts, thereby eliminating the need for manual intervention and reducing the risk of disputes. The study examines key architectural layers, interoperability challenges, and practical implementation strategies, supported by contemporary case studies from 2021 to 2025. Experimental analysis and industry examples highlight substantial improvements in traceability, security, fraud prevention, and operational efficiency. This scalable and secure approach addresses persistent challenges such as counterfeiting, delays, and lack of trust, paving the way for resilient, data-driven, and future-ready supply chain ecosystems.
