Clinical case describes treatment challenges of acute kidney injury in a comorbid patient with hemorrhagic fever, suggesting tailored therapeutic approaches.
A clinical case is presented to your attention. The purpose of the article is to describe a clinical case of treatment of a comorbid patient with severe hemorrhagic fever with renal syndrome, acute kidney injury caused by acute tubulointerstitial nephritis in the presence of a number of concomitant diseases. Lack of alertness regarding the initial period of hemorrhagic fever with renal disease syndrome leads to severe complications. The generalized nature of the infection with the involvement of various organs and systems in the pathological process determines the polymorphism of symptoms. The most important tasks are to determine the tactics of managing a comorbid patient in combination with a severe course and to study the links between the clinical course and comorbidity. The research material was the patient's anamnesis, the results of laboratory and instrumental studies and consultations of specialists of various profiles. Therapeutic tactics in conditions of comorbidity require well-designed therapy. In such situations, the order of treatment is determined individually, taking into account the assessment of the level of progression and severity of diseases. The generally accepted treatment method is pathogenetic therapy aimed at reducing intoxication, restoring circulating blood metabolism, improving rheology and microcirculation, reducing vascular permeability, and normalizing the acid-base state. The development of drugs for effective antiviral therapy of hemorrhagic fever with renal syndrome remains a very important and unresolved problem today, since drugs with non-specific effects on the infectious process are still used in widespread clinical practice. At the same time, the presence of concom
