Lipodystrophy is a common disorder associated with human immunodeficiency virus infection and its therapy. Lipodystrophy is also an acquired or inherited disorder known to be associated with insulin resistance and lipid abnormality. Characteristically it is associated with abnormal fat deposition or fat loss. Here, we present a case of lipodystrophy without insulin resistance or lipid abnormalities, as well as no family history of the condition. The absence of insulin resistance with characteristic fat loss and low C3 with normal C4 clinched the diagnosis.
