Introduction: epidermoid cyst or sebaceous cyst in external auditory canal is a rare clinical entity. A review of the English literature fail to show a detailed excision procedure for this clinical entity. We are presenting here with such a rare condition of external auditory canal of a middle aged female. Case Report: 56 year old female patient attended with complain of ear blockage and irritation of 6 months duration. Necessary clinical and radiological investigations revealed well encapsulated soft tissue mass with minimal enhancement involving EAC. Management: it consisted of total excision of the cyst with intact wall. HPE revealed it to be an epithelial lined cyst filled with laminated keratin located within the dermis. Conclusion: it is a rare case of epidermoid cyst of EAC, excised completely via modified endaural approach.
