Retrospective observational study reports safety and treatment response in elderly patients with DLBCL, suggesting significant toxicity risks.
A BSTRACT Background: The aim of this study was to evaluate treatment response and the incidence of adverse events in elderly patients with relapsed or refractory diffuse large B-cell lymphoma treated with polatuzumab vedotin. Methods: A retrospective, observational study was conducted. Clinical variables and treatment response were assessed using the Lugano classification. Multivariate analysis was used to identify predictors of hematologic toxicity. Results: Twenty-three patients were analyzed. The complete response rate was 43.5%, with 34.8% experiencing early progression. Grade 3–4 neutropenia occurred in 47.8%. One-year mortality was 43.5%. Conclusion: Polatuzumab vedotin showed moderate efficacy but was associated with significant hematologic and infectious toxicity. Risk stratification could help optimize outcomes.
