Case report demonstrates improved electrophysiological outcomes in bradyarrhythmia using dual-site pacing.
We report a clinical case of successful dual-site physiological pacing, integrating Left Bundle Branch Area Pacing (LBBAP) and Bachmann Bundle Pacing (BBP). This procedure utilized Medtronic SelectSecure 3830 leads with C315 and C315S5 delivery sheaths for LBBAP and BBP, respectively. The procedure resulted in excellent electrophysiological outcomes: the QRS duration (QRSd) was reduced from 114 ms to 98 ms with a left ventricular activation time (LVAT) of 61 ms post LBBAP. The P-wave was shortened from 109ms to 88ms along with electrophysiological parameters indicating successful Bachmann bundle capture. This case report demonstrates the successful deployment of dual site physiological pacing with achievement of desired electrophysiological endpoints in a patient with bradyarrhythmia and history of atrial fibrillation.
