A woman with a history of surgery for secundum atrial septal defect repair was admitted to our hospital with acute heart failure. Echocardiography revealed hypoplasia of the posterior mitral leaflet and persistent dilation of the right heart chambers. Cardiac magnetic resonance imaging confirmed the presence of an inferior sinus venosus atrial septal defect that had previously been undetected.
