Analysis of the Casual Factor for not Carrying out the Destruction of Inactive Medical Record File at the Muhammadiyah Lamongan Hospital

Destruction is the act of physically destroying complete medical record archives that have ended their function and use value.The purpose of the study was to analyze the factors that influence the non-execution of destruction. The research method used is descriptive qualitative, there are 3 informants using 5M which is described through a fishbone diagram, namely man, method, machine, material, money. The results of the study obtained man, namely the last education of medical record filing officers is high school, special training for destruction has not been carried out, and the lack of knowledge of officers, method, standard operating procedures for destruction do not exist, material, storage shelves are still minimal and there is no retention schedule, machine, there is no scanner, money, a special plan for the destruction budget has not been made.
