Triplet pregnancies are rare, and monozygotic triplet pregnancies are even rarer. Triplet pregnancies present increased risk in every aspect of the prenatal course. The rate of triplet and higher order births, in the United States, was 78.9 per 100 000 and that rate has decreased from a peak, in 1998. Sonography plays a vital role in the diagnosis and management of all multifetal pregnancies. To further illustrate the importance of sonography, a case of spontaneous monochorionic triamniotic (MCTA) triplets is presented. This set of triplets were delivered at 32 weeks gestation without significant adverse neonatal outcome.
