Chronic inflammation, bronchiectasis, and tumors are more likely to cause hemoptysis.Hemoptysis triggered by trauma from 39 years ago is extremely unusual, which must be monitored during the diagnosis and treatment of hemoptysis, especially for individuals with various causes of hemoptysis.A case of a 76-year-old patient with hemoptysis who did not respond to medical therapy was reported. He developed chronic bronchitis and has smoked for a long time.In addition, he suffered right clavicle and 1st-2nd rib trauma 39 years ago.The bilateral bronchial arteries were normal on digital subtraction angiography (DSA), while the pulmonary artery shunt was seen in the right intercostal artery and right superior thoracic artery. The abnormal artery was embolized, and there was no recurrence of hemoptysis for a year.
