Abstract Diabetes and malignant tumors often lead to abnormal immune function, increasing susceptibility to herpes zoster infection and severe post-herpetic neuralgia. Renal insufficiency following renal cell carcinoma surgery can be compounded by nephrotoxic antiviral drugs. A 68-year-old man developed herpes zoster at the surgical site and severe neuralgia after left nephroureterectomy for left kidney clear cell carcinoma, which had resulted in postoperative renal insufficiency. Brivudine therapy suppressed infection, and relieved neuralgia, without affecting renal function. Short-term control of herpes, crust formation, and neuralgia relief were achieved by day 14. Nephrotoxicity should be considered when selecting antiviral drugs for such patients.
