Sternal fractures are rare injuries caused by blunt trauma. There is no clear consensus regarding the surgical treatment of sternal nonunion. We report a rare case of thoracoscopically assisted osteosynthesis using a mesh plate for sternal nonunion. The patient was a 53-year-old male with a sternal fracture with multiple rib and thoracic compression fractures caused by a traffic accident. Despite conservative treatment, the sternal fracture resulted in nonunion. Thoracoscopically assisted osteosynthesis was performed using a mesh plate secured with sufficient locking screws and autogenous bone grafting. Following surgery, the bone was successfully fused and the symptoms disappeared. We encountered a case of sternal nonunion with multiple rib fractures and thoracic kyphosis due to a Th3 compression fracture. We successfully performed thoracoscopically assisted osteosynthesis using a mesh plate and safely inserted more screws and improved fixation. This approach was effective in achieving proper fixation while avoiding damage to the nearby organs.
