Introduction Cardiac complications of SARS-CoV-2 infection have had a significant impact on quality of life and work in affected individuals. COVID-19 vaccines have reduced case fatality, but side effects have been reported. Methods We present the case of a physician with a history of arrhythmogenic cardiomyopathy following viral myocarditis. After administration of the first dose of COVID 19 mRNA vaccine, she presented suggestive symptoms for acute heart failure. In the absence of any other diagnostic evidence to explain the myocardial damage, a causal relationship with vaccination was established. Cardiac function was periodically assessed by evaluating myocardial strain parameters. She subsequently suffered two SARS Cov 2 infections, without significant heart damage. Results and discussions The impact on health has had a significant effect on working life, physically demanding work (operating theatre, night shifts) could not be performed until health improved. In addition, regular medical visits and intermittent health-related absences affected the overall attendance at work and the work schedule. Conclusions This case highlights the importance of careful risk assessment and monitoring of employees with chronic diseases in occupational health services. The particularity of this case is the existence of a post- viral cardiomyopathy, subsequently decompensated in the context of vaccination, which required work-life adjustments. Healthcare institutions and policy makers should be aware of these potential implications and consider appropriate adaptations for healthcare workers in the context of the emergence of chronic diseases and the long-term effects of SARS - CoV2 infection. Ethics approval The study was approved by the Institutional Ethical Committee of Clinical Emergency Hospital for Children Iasi.
