Aim Following one of the first cases of Zinner syndrome associated with inguinal hernia, aim to review existing evidence and literature for treatment around this, especially non-surgical treatment. Patient A 68-year-old male who was investigated for raised PSA. As part of the work up, he was found to have an incidental finding of right inguinal hernia associated with a large contralateral seminal vesicle cyst and an absent kidney. Results For surgical candidates, we would recommend open hernia repair. Bladder resection is rarely indicated. For non-surgical candidates, there is no consensus on how to treat their condition. Conclusions The patient was treated conservatively with an alpha blocker to manage outflow obstruction. Further non-surgical treatment could involve a 5 alpha reductase inhibitor or catheterisation. Further research is needed.
