A 45-year-old female patient presented to us with a severe headache and vomiting. Neurologically, she was intact. Her MRI brain was done, which showed a lesion in the pineal region which showed restrictions on diffusion weighted images suggestive of an epidermoid cyst. She underwent surgery, a suboccipital craniotomy in a sitting position with a supracerebellar infratentorial approach, endoscopically to completely excise the tumour. The endoscopic approach provided a direct end to vision through a small craniotomy. We could also view and completely excise flakes of epidermoid cysts from hidden areas. The post-operative course was uneventful, and she was discharged without any deficits. A post-operative CT scan of the brain showed complete excision of lesion.
