Dystocia due to lateral deviation of head and neck is one of the most common types of ruminant dystocia. In one such case, a 7-year- old Punganur cow was reported and successfully managed. The cow was straining and showing symptoms of labor pain and respiratory distress. The cervix was fully dilated, and the birth passage was dry. Foetus was in anterior presentation, dorso-sacral position with extreme left deviation of head and neck. When attempts made by extraction to expel out the foetus, Punganur calf was delivered. The cow was provided with proper postoperative care and management to facilitate the recovery of the uterus and restore normal functioning.
