Background: Systemic sclerosis (SSc) is chronic autoimmune disorder characterized by vasculopathy, dysregulated immune system leading to inflammation, and fibrosis. Resolvins, belonging to the specialized pro-resolving mediators, are produced as a response to acute inflammatory process in termination and resolution of inflammation [1,2] Objectives: to assess the levels of serum Resolvins (Rv) in SSc patients and to investigate the relationship between the Rv levels and clinical features and organ involvements. Methods: 40 SSc patients and 40 healthy healthy controls (matched for age and gender) were included in the study. The clinical features and organ involvements of SSc patients were recorded. The level of precursors of Rvs including docosahexaenoic acid (DHA) and eicosapentaenoic acid (EPA), RvD1, RvD2, RvE1 and RvE2 were analyzed with ELISA from sera of patient and control group Results: The mean age of patient and control group were 48.53±13.14 and 46.30± 13.5 years, respectively (p=0.46). The median disease duration of SSc patients was 5 years and 65% of patients had lcSSc (Table 1). The DHA serum level was similar in patients and controls: however, in SSc the median level of serum RvD1 resulted significantly higher 265.20 (291.10) pg/mL when compared to controls (127.95 (105,77)) (p<0.001). Similarly, the median level of serum RvD2 from SSc patients (258.20 (233.80) was significantly higher compared to controls (120.30 (155.70) pg/mL, p<0.001) (Table 2). In controls, the median level of serum EPA (387.80 (255.90) was markedly higher than in SSc patients (274.45 (123.95) ng/mL, p=0,03). However, the levels of RvE1 and RvE2 were similar in both groups. The evaluation of Rvs levels in comparison with presence of organ involvements, including interstitial lung disease, cardiac, renal, musculoskeletal, and gastrointestinal involvement, revealed no significant association between the level of these mediators and organ involvements. Similarly, there was not statistically difference in the level of mediators between lcSSc and dcSSc. Conclusion: The present data may suggest that the increased level of RvD1 and RvD2 in SSc patients reflect the response to the dysregulated and exaggerated inflammatory process induced by the alteration of the immune system in SSc. In the future, RV levels should be investigated in larger cohorts of patients to verify their role in the disease and the correlation with organ involvement.
