Co-administration of Clopidogrel-based DAPT and Antisecretory Agents: Comparison of Incidence of Cardiovascular Events

Background : Patients who have undergone percutaneous coronary intervention (PCI) receive clopidogrel-based dual antiplatelet therapy (DAPT) to prevent complications.Concurrent administration of proton pump inhibitors (PPI) is common to prevent gastrointestinal side effects associated with this therapy.While esomeprazole, a substrate for CYP2C19, inhibits the conversion of clopidogrel to its active form, tegoprazan, primarily metabolized by CYP3A4, provides a potential advantage.Therefore, this study aims to compare the incidence rate of cardiovascular events between esomeprazole and tegoprazan in patients receiving clopidogrel-based DAPT after PCI at a tertiary hospital.Methods : This retrospective study analyzed the electronic medical records of patients pre-이희원 : 관상동맥 스텐트 삽입술 시행 후 Clopidogrel 기반 이중 항혈소판 치료를 받는 환자에서 Esomeprazole과 Tegoprazan 병용 및 심혈관계 사건 발생 비교 관상동맥 스텐트 삽입술을 시행한 환자들은 스텐트 재협착 부작용을 예방하기 위하여 항혈소판 치료를 시행하며 현재는 clopidogrel과 aspirin의 병용요 법이 가장 일반적으로 이용되고 있다. 1),2)Clopi-dogrel과 aspirin의 이중 항혈소판 치료 시 위장관 출혈을 방지하기 위해 PPI (Proton Pump Inhibitor)가 함께 처방되는 경우가 많다.Clopidogrel 은 cytochrome (CYP) P450 system에 의해 생 성된 활성 대사체가 혈소판의 P2Y adenosine diphosphate 수용체에 비가역적으로 결합하여 혈소 판 응집을 억제하는 약물이다.CYP2C19이 활성형 의 형성에 주요한 역할을 하므로, CYP2C19의 기질 과 clopidogrel 간의 상호작용이 발생할 수 있다. 3) PPI는 CYP2C19의 기질로 clopidogrel과 병용 시 clopidogrel의 활성형 전환을 억제하여 항혈소판 효 과를 감소시킬 수 있으며, 3),4) 이를 뒷받침하는 여러 연구들이 보고되었다. 5)-7)
