Ischemic cerebrovascular disease is a relatively serious clinical disease with high morbidity and mortality every year. Clearly identifying the cause of stroke in patients, timely assessing the ischemic penumbra and collateral circulation status, is extremely important for the treatment and prognosis of stroke patients. Arterial spin labeling technique can non-invasively and quickly obtain information about cerebral tissue perfusion, and has the advantages of not requiring contrast agents, easy operation, and repeatability. It can provide clinicians with abnormal perfusion area of patients, and be used for early diagnosis, disease monitoring and prognosis assessment of patients. This article provides a comprehensive review of the arterial spin labeling technique, covering its fundamental principles, technological advancements, selection of key parameters, applications in ischemic stroke, and assessment of collateral circulation.
