Blunt cardiac rupture remains a rare yet life-threatening consequence of trauma, often presenting diagnostic challenges and requiring prompt intervention for favourable outcomes. We present a case of a 15-year-old male with a firearm injury to the upper abdomen, ultimately leading to the discovery of blunt cardiac rupture.Despite initial stability, the patient's clinical course revealed complexities necessitating multidisciplinary evaluation. The identification of a bullet shell lodged in the left atrial appendage, causing mitral valve damage, underscores the need for meticulous assessment and surgical intervention. This case underscores the critical importance of recognizing and managing blunt cardiac rupture in the context of traumatic injuries, emphasising the necessity for a comprehensive approach to optimise patient care.
