Radiocephalic AVF is commonly the primary vascular access for hemodialysis. However, complications such as outflow vein stenosis or occlusion often occur with long-term use, leading to a high negative pressure during hemodialysis.Endovascular angioplasty is an effective and safe treatment for stenotic or occluded outflow veins; however, it may pose difficulties in cases of complete occlusion, particularly if anatomical access is challenging. This case study describes the successful creation of a venovenous fistula using a sharp recanalization technique to address a total occlusion of the cephalic vein in a patient with radiocephalic AVF, resulting in a well-functioning fistula during follow-up.
