Purpose. To study the possibility of predicting the minimum effective exposure time of autologous conditioned plasma (ACP) in surgery of macular hole on the retinal surface, according to the Thrombodynamics test. Material and methods. 32 patients diagnosed with primary perforating macular hole were operated on. To assess the state of plasma hemostasis and analyze the spatiotemporal dynamics of fibrin clot growth, all patients underwent the Thrombodynamics test. Platelet-rich ACP was obtained using ArtrexACP technology. Patients underwent vitrectomy using the inverted flap technology of internal limiting membrane. After completely replacing the BSS with air and drying the retina, 50 μl of ACP was applied to the area of the retinal rupture. In the postoperative period, according to iOCT data, the time of onset of fibrin polymerization in the rupture zone was studied. The obtained data in seconds was compared with the Lag time indicator of the Thrombodynamics test. Results. As a result of surgical treatment, closure of the macular hole was achieved in all patients. The obtained statistical data, in our opinion, indicate the similarity of the processes simulated in the Thrombodynamics test with the processes of initiation of fibrin polymerization on the surface of the retina in the area of the macular hole. Conclusion. The use of the Thrombodynamics test at the stage of preoperative examination is promising. The results of this study may help optimize surgical treatment of macular holes using ACP and reduce its duration.
